If you’ve been asked to provide blood pressure readings, you can do so here.

Blood Pressure Review Form.

This form is for you to submit blood pressure readings.

To enable us to analyse your blood pressure accurately please remember to: 

  • Sit quietly without distraction whilst taking your blood pressure 

  • Take your blood pressure for 7 days, once in the morning and once in the evening 

  • On each occasion take your blood pressure twice, at least one minute apart 

Then, return the blood pressure machine and this sheet to Haresfield reception desk. The GP will then access your readings and contact you with any concerns or queries.
